That's happened to me a few times. I fixed mine by sticking the wire back in place and sewing up the hole where it poked through. If the cloth is really worn, it can be patched up with a little piece of scrap material.
It also depends on the ends of the underwire - some of them have a little plastic cap, which can wear/fall off, and makes it pretty much impossible to stop the metal cutting any new fabric you patch over.
Sure. 1) Cover the exposed end of the underwire with a little bead of hot glue, to "pad" the pokey end of the wire, and let the glue cool. (I don't do this part, but maybe it's a good idea?) 2) Shove the underwire as far as you can back inside of its casing. 3) Take a piece of ribbon or bias tape. Fold it over three times. 4) Pin the ribbon in place so it covers the hole in the casing, but (hopefully) doesn't protrude to show from the front of the bra. 5) Sew the tripled piece of ribbon/bias tape over the hole in the casing. She says to whipstitch all around the edge, but I say use whatever hand-sewing stitch you want!

This is more or less the technique I use, although I skip the hot glue part.
